§ 12-1072. Application; contents.

Latest version.

Any person desiring to operate a food establishment shall make written application to the building official for a permit, license, or certificate on forms provided by the city. Such application shall include the name and address of each applicant, the location and type of the proposed food service the location and type of the proposed food service establishment, and the signature of each applicant.
